  • Preparing meals and snacks according to instructions

Qualifications:

  • High school graduate or G.E.D. certificate

Job Summary

 Within the framework of the objectives and policies established by the Company, plans, coordinates and monitors home care services with clients and the office staff. Participates as a member of the company’s management team in planning, formulating policy, and ing istrative decision making with an emphasis on care quality and caregiver preparedness. In summary, is responsible for client care, caregiver training, and oversight of the Quality Improvement Program.

 Essential Functions

 ·        Maintains compliance with all care-related agency policies, procedures and regulatory requirements

·        Evaluates client status on a 60 day cycle, reviews and coordinates care plan with client needs

·        Reviews care staff for conformity with policies and procedures, care standards, state regulations and home care requirements

·        Evaluates the appropriate assignment of personnel to meet the specific need of individual clients and coordinates with office staff

·        Ensures the quality of patient care and service by using specialized knowledge and judgment

·        Performs care management activities for the individual clients, including but not limited to assessments, status evaluations, client, family and care staff teaching

·        Ensures that changes in the client’s medical or mental condition are reported to the client’s family, healthcare professionals and to Director of Operations.

·        Maintains visit records in accordance with Agency policy as well as state regulations as applicable

·        Assists in providing orientation and in-service education for Personal Care Assistants and Homemakers

·        Participates in quality improvement activities and evaluations of the Agency.

·        Investigates problems, irregularities, reported incidents, policy violations and other reportable events implementing the appropriate corrective action and follow-up.

·        May be required to travel on company business.

·        Participates in after-hours call rotation.

 

Additional (non-essential) Functions

 ·        Promotes Agency to the community, participating in community programs with other professionals when possible.

·        Other duties as assigned by the Director of Operations and Owners.

 

Education, Experience, Knowledge, Skills, Abilities and Availability

 ·       Graduate of a State approved school of vocational nursing with 6 months of satisfactory nursing experience

·       Currently licensed without encumbrances and in good standing as an LVN in the state of California.

·       Meets the health requirements for employees providing direct client care

·       Read, write, speak and understand English as needed for the job

·        Has a valid driver’s license

  • Preparing meals and snacks according to instructions

Qualifications and Skills:

  • High school graduate or G.E.D. certificate
  • Must be able to lift/move 40 lbs without assistance 
  • Possess a valid driver’s license and mode of transportation
  • state mandated background check
  • a pre-employment phyisical and drug screen testing
  • Have a current TB test
  • Be willing to drive a 15-miles radius to client's home

The ed nurse plans, organizes and directs hospice care services specific to each patient/family which includes instruction and evaluation in the home. The professional nurse is able to the Director of Patient Care Services for implementing the patient plan of care and communicating each patient needs to the Interdisciplinary team. PRIMARY FUNCTION

The issions ed Nurse is an Interdisciplinary Team member whose primary responsibility is to present ANCHOR HEALTH services and philosophy to patients and families in a manner that results in the ission of appropriate patients.

JOB RESPONSIBILITIES FOR THE ISSIONS ED NURSE

1. issions ed Nurse participates in the information gathering, analysis, education, and form completion aspects of the issions process

2. issions ed Nurse evaluates patients for hospice appropriateness, e.g. medical records review as requested.

3. issions ed Nurse Obtains agreement for ission of referred patients and families who meet ission criteria.

4. issions ed Nurse analyzes data collected by the ission staff or ANCHOR HEALTH clinicians to facilitate/obtain ission orders

5. issions ed Nurse builds effective referral source relationships to the high quality and responsiveness expectations associated with ANCHOR HEALTH.

6. issions ed Nurse maintains clinical and professional competence.

7. issions ed Nurse takes the initiative in developing effective relationships internally with the appropriate issions and patient care resource persons.

8. issions ed Nurse utilizes interpersonal skills to help appropriately educate and address any questions the patient and/or patients’ family has concerning hospice and palliative care.

9. Other duties and responsibilities as assigned

JOB REQUIREMENTS FOR THE ISSIONS ED NURSE

1. issions ed Nurse has knowledge of the principles of the intake, ission, and transition to care process with the ability to further develop skills in these competency areas.

2. issions ed Nurse has the ability to communicate effectively with patients and families, physicians, referral sources and colleagues.

3. issions ed Nurse has the knowledge of clinical manifestations of diseases and ability to develop ANCHOR HEALTH specific clinical knowledge.

4. issions ed Nurse has the ability to use the ANCHOR HEALTH information system and mobile technology platforms.

5. issions ed Nurse has reliable means of transportation.

6. Willing to travel to both Sacramento and Solano County for issions.

ESSENTIAL FUNCTIONS/PHYSICAL DEMANDS

The physical demands of the issions ed Nurse are representative of those that must be met by an employee to successfully perform the essential functions of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

While performing the duties of the issions ed Nurse, the employee is regularly required to:

1. Assist in lifting and transferring patients, as required.

2. Lift objects in excess of 20 pounds

3. Stand or walk in excess of one hour per day

The issions ed Nurse will be required to sit and stoop, kneel, and crouch.

DIRECTION RECEIVED

The issions ed Nurse reports to the issions Manager designated in the location. In the event the location does not have an issions Manager, the issions ed Nurse will receive direction from the Patient Care or program designee

QUALIFICATIONS FOR THE ISSIONS ED NURSE

1. Graduate of a National League of Nursing accredited school of nursing.

2. Current licensure in State of CA and R certification.

3. ed nurses shall have a minimum of one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing from a program accredited by the National League of Nursing and a current RN license.

Minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred.

4. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. issions ed Nurse is expected to have strong presentation skills.

5. Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching.

6. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
