Principal Responsibilities
The Occupational Therapist performs treatments and exercises to help patients recover from illnesses and injuries. Their main duties include using various treatment techniques to develop personalized treatment plans, prescribing medications to relieve pain and increase mobility and teaching patients exercise techniques to improve their health.
This position reports to the Director of Rehabilitation Services.
· Consult with patients to learn about their physical condition and symptoms. Review their medical history and referrals from doctors or surgeons.
· Diagnose movement dysfunction by listening to patients and observing them as they move.
· Develop individualized treatment plan
· Coach patient and teach them therapeutic massage to ease patients’ pain and provide stimulation to promote healing

· Utilize standardized assessments and evidence-based practice to clinical interventions.

· Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
· Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
· Effectively communicates with supervisor and other health team regarding patient progress, barriers, and treatment plans.
· Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
· Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy. Accurately records treatment minutes for services provided per all regulatory and clinical requirements.
· Participate in facility and company required trainings, in-services and conferences.

Principal Responsibilities
• Direct all aspects of quality patient care, including evaluation and treatment, treatment planning, goal setting, family education and documentation according to the company guidelines, professional standards and community needs.
• Provides professional treatment while using therapeutic techniques for the rehabilitation of patients with speech, language, hearing, oral motor, swallowing and cognitive disorders.
• Plans activities and provides therapeutic services for speech and language disorders.

· Prepare accurate and timely case studies for referrals to psychologists, school nurses, law enforcement, and other agencies; make referrals to outside agencies to provide students access to high quality options and a variety of activities.
· Assist students in the transition process between elementary, middle, and high school and those returning from alternative placements.
· Coordinate, establish collaborative relationships and enhance communication between school, families, community agencies and students; participate in identifying and resolving school issues involving attendance; consult with teachers to encourage open communication with students; actively participate in identifying community problems and developing needed community services.
· Plan, develop, organize, and actively participate in staff development, parent meetings and workshops to provide consultation and information on resources to meet the needs of students, families, and staff; develop and maintain current resource information.

EDUCATION AND EXPERIENCE:
· Any combination equivalent to: graduation from an accredited college with a Master’s degree in Social Work, Counseling or related field.
· Case management experience preferred.
