Minimum Requirements:

Treats patients and their families with respect and dignity. Coordinates and directs patient care to ensure patients’ needs are met and that physcian orders and facility policy is followed.

Demonstrates ability to assess patient, his/her need, and ability to tolerate treatment and reassessment for response to treatment.

 Demonstrates the ability to deal with pressure to meet deadlines, to be accurate, and to handle constantly changing situations.

 Demonstrates the ability to deal with a variety of people, deal with stressful situations, and handle conflict.

 Attends and conducts education inservice programs to maintain professional competency. Obtains appropriate program-specific education and training.

 Performs all additional duties as assigned by the department manager.

We are a home care agency that has been in business since 2007 and in need of a person to perform in-home assessments of elderly clients in the Fresno, CA region (clients located within a 2 hour drive of Fresno, CA). $40/hour Per Diem. 

 

The assessment consists of talking to the client in person at their home - and asking a few questions about their non medical care needs (i.e. Bathing, dressing, grooming, light house cleanning, etc) and filling out a number of health care assessment documents.

 

The assessments take 30-60 minutes. Travel time is paid from the person's home and back at $40/hour. Flexible schedule. Can work anytime from 8am to 8pm Monday through Sunday. Weekends only is acceptable. Great part time job for extra money on the side of your regular job. Experience with assessments required. Will train on the job.

 

We just started a contract with the Department of Veterans Affairs so there is not a big need right now - but as we get more clients in the program the need will grow. When we have need of an assessment we will reach out and you would need to get the assessment completed within 3-5 calendar days of getting notified of the new assessment, as this assessment will determine the number of hours the veteran will get approved for care each week. 

Must get a TB (Tuberculosis) skin test if not had one recently. You can go to your local County Health Department and walk in to get a skin TB Test. Just provide your ID and tell them it is for employment. It costs $25. You will return 48-72 hours later to have it checked. POSITION DESCRIPTION

RESPONSIBILITIES

• Promotes healing of acutely ill residents whose cardiac or pulmonary systems have been compromised through illness or injury through the provision of respiratory care, diagnostic testing, psychosocial needs and resident and/or responsible party education.

• Provides proper and safe istration of respiratory care including intermittent and continuous therapy, mechanical ventilation, airway management and non-invasive oxygen and carbon dioxide monitoring of residents.

• Provides the set-up, istration and therapeutic use of medical gases and istration apparatus, environmental control systems, humidification, aerosols, medications, ventilatory , airway management, chest physiotherapy as they relate to the patient in the home environment.

• Follows the clinical management of the rehab department, including oversight related to case management, quality improvement, care planning, clinical utilization and resident identification.

• Ensures the clinical manage of rehab department including reimbursement, criteria services, billing, resident records and equipment records.

• Collaborates with nursing leadership to integrate respiratory care functions into day to day nursing practice as appropriate; trains other personnel as needed.

• Assesses, plans, implements and evaluates respiratory therapy care and life to residents in all areas of the nursing center; Encourages progression and enhances efficient operation of respiratory services.

• Manages effective coordination of equipment by staff; Ability to coordinate, receive and document the order for equipment and/or services from referral source, physician, resident/client, family member, or care giver as needed; Assesses the need of the residents and the best choice of equipment and/or services to meet those needs.

• Coordinates communication with residents and/or responsible parties, physician, and nursing on changes in resident’s condition and/or or needs and carries out adjustment to the resident’s care plan/plan of treatment.

• Develops and implements discharge plans for residents.

• Tracks and maintains follow up on preventative maintenance and scheduled visits for respiratory therapy equipment.

• Assess developmental needs for all age groups across the life span and adjust care and instruction with regard to the physical, psychosocial, and emotional needs of the resident.

• Responsible for the provision of general medical equipment and supplies to the company’s resident population, as ordered by a physician.

• Provides professional treatment while using therapeutic techniques for the rehabilitation of patients with speech, language, hearing, oral motor, swallowing and cognitive disorders.

• Document patient evaluations, treatment progress notes, and discharge summaries for the patient’s medical records in a timely manner as defined by the department policy manual.

• Providing a full range of speech therapy modalities consistent with previous education and expedience.

• Assist with the clinical affiliations of speech therapy assistants.

• Perform a clinical assessment to identify patient needs and appropriateness of patient. Analyze patient needs and ister appropriate care as ordered by a physician.

• Document, prepare and complete a Plan of Care, progress notes, phone orders and other clinical record documentation on an ongoing basis.

• Ensures the delivery of quality rehab services.

• Demonstrates courtesy, comion, and respect to patients, caregivers, customers, physicians, and co-workers.

• Maintain Compliance with Compliance Program and plan, adhere to the Company’s Code of Conduct.

DEBERES Y RESPONSABILIDADES GENERALES: GENERAL

Position Description

A PTA is responsible for assisting Physical Therapist in helping patients recover and rehabilitate from the effects of a serious illness, injury or medical event, regaining their own best possible level of functional ability, well-being and independence. Provides services to improve strength and function, restore more efficient movement, improve flexibility and aid patients in restoring mobility and/or improving balance and gait after hip fractures and other orthopedic injuries, medical illness, or as they recover from t replacement procedures.

Principal Responsibilities

• Provide care of residents who seek short-term/transitional care with the goal to recover and rehabilitate from the effects of serious illness, injury or medical event, regaining their own best possible level of functional ability, well-being and independence.

• Assist in providing appropriate aspects of quality patient care, under the supervision of licensed therapists.

• Document all services provided to ensure accurate and timely billing of therapy services.

• Assures that patient care follows the established Plan of Care, the organization s guidelines, meets professional standards, community needs and is in compliance with Federal and State guidelines.

• Under the supervision of a Physical Therapist, identifies and develops a comprehensive plan of care; Develops and implements individualized therapy programs designed to restore, reinforce and enhance performance.

• Identify need for and makes referral to other disciplines to address the comprehensive needs of the patient under the supervision of a Physical Therapist.

• Other duties, responsibilities and activities may change or assigned at any time with or without notice.

Qualifications

• High School Diploma required

• Requires valid Physical Therapy Assistant license or certification in the state of practice, or proof of license eligibility.

This is a Temporary full time (40 hours a week) starting on 5/28 for 3 months and then can stay as PRN or part time afterwards.

POSITION DESCRIPTION: To provide direct patient care and activities which build and maintain the outstanding outcomes of the therapy department.

ESSENTIAL JOB FUNCTIONS:

Provides patient care under the direction of a physical therapist. Completes documentation according to established procedures. Is involved in duties as assigned. Contributes to professional development of self and others. Enhances interdisciplinary team through professional skills and knowledge. Demonstrates safe working practices.

Provides patient treatment which meets patient needs in accordance with the plan of care, and uses current treatment knowledge in accordance with facility clinical procedures. This includes but is not limited to:

Provides comprehensive, appropriate patient treatment in accordance with patient’s individual treatment plan as stipulated by supervising Physical Therapist.

Providing clear instruction to patients and families.

Communicating with supervisor and other health team regarding patient progress, problems, and treatment plans.

Instructing patients’ families or nursing staff in specified follow through programs.

Demonstrating knowledge of appropriate resources for problem-solving and clinical best practice.

Writes accurate, complete, and clear documentation in accordance with regulatory, licensing, payor, and accrediting agencies. This includes but is not limited to:

Recording treatment charges accurately and insuring that documentation is consistent with billing data.

Writing clear and effective daily notes and progress notes.

Assists with maintenance of patient charges and istrative . This includes but is not limited to:

Completing charges accurately on Productivity and Activities Log daily.

Assisting therapists with scheduling and appointments for patient services.

Consistently demonstrates facility’s interdisciplinary approach to patient care. This includes but is not limited to:

Actively participating in patient care conferences.

Collaborating with and informing facility staff in areas of patient progress.

Educating of the team in areas of expertise.

Working with facility to teach documentation practices, which assist the therapy team.

Communicating effectively with discharge planners.

Integrating input from patient and family into treatment sessions.

Contributes to professional development of self and others. This includes but is not limited to:

Utilizing continuing education assistance to achieve targeted quality improvements.

Interacting with peers at local acute hospitals and other environments, if appropriate.

Presenting at weekly rehab team conferences and facility in-services.

Maintaining current body of knowledge in areas of clinical expertise through seminars, professional journals, and peers.

Meeting continuing education requirements of regulatory agencies.

Aids in building and maintaining a positive facility culture as a professional member of the staff. This includes but is not limited to:

Consistently demonstrating positive core values in interactions with co-workers, patients and families, with all other persons involved with the patient’s care, and the general public.

Consistently demonstrating professional standards.

Contributing to a positive work team by sharing information, using problem-solving methods and accepting new ideas, constructive criticism, or advice from others.

Using proper body mechanics during treatments of patients and during activities.

Wearing appropriate safety equipment.

QUALIFICATIONS:

Skills, Knowledge and Abilities:

Knowledge of Physical Therapy treatment techniques.

Understanding the principles of physical modalities employed in Physical Therapy.

Knowledge of use of prosthetic devices, ambulatory aids, assistive and resistive devices, and training.

Knowledge of medical terminology.

Ability to work with all types and levels of patients.

Must have good written and verbal communication skills.

Education:

Associate of Science Degree from an accredited Physical Therapy Assistant program.

Experience:

• Experience in geriatric rehabilitation preferred.
• Experience in adult neurological, cardiac, and/or general rehabilitation is preferred.

Physical Requirements:

For Direct Patient Care:

Must be able to lift and transfer patient to and from all surfaces a minimum of 10 times per day.

Must be able to lift approximately 50-70 pounds a minimum of 10 times per day.

Must be proficient with the use of hands for repetitive grasping and fine motor manipulations.

Must be able to perform R, if required.

Physical Therapy Assistant will spend approximately 60-80% of time bending, stooping, kneeling, and reaching while working with physical therapy patients.

Physical Therapy Assistant will spend approximately 60-80% of time walking/standing.

For Activities:

Physical Therapy Assistant may spend approximately 10-20% of time sitting at desk or terminal entering data.

License/Certification/Registration:

Current Physical Therapist Assistant Registration/State Licensure as required

Responsabilidades principales

El fisioterapeuta realiza tratamientos y ejercicios para ayudar a los pacientes a recuperarse de enfermedades y lesiones. · Diagnosticar la disfunción del movimiento escuchando a los pacientes y observándolos mientras se mueven.

· Desarrollar un plan de tratamiento individualizado

· Entrenar al paciente y enseñarle masajes terapéuticos para aliviar el dolor de los pacientes y proporcionar estimulación para promover la curación

· Mantener el registro del paciente haciendo un seguimiento de los objetivos

· Utilizar evaluaciones estandarizadas y prácticas basadas en evidencia para apoyar las intervenciones clínicas.

· Evalúa el progreso del paciente y ajusta los tratamientos en consecuencia en el registro clínico según los requisitos reglamentarios y de práctica clínica.

· Evalúa los resultados del tratamiento para la eficacia. Los servicios se modifican en relación con la evaluación del paciente y las referencias se realizan en relación con los resultados.

· Brinda consulta y consejería a pacientes, así como a familiares, cuidadores y otros prestadores de servicios relacionados con los trastornos físicos.

· Se comunica de manera efectiva con el supervisor y otros del equipo de salud con respecto al progreso del paciente, las barreras y los planes de tratamiento.

· Proporciona un resumen completo del alta de los servicios prestados según los requisitos normativos y clínicos.

· Documenta toda la información reglamentaria y clínicamente requerida en el sistema de documentación electrónica y mantiene registros de pacientes de acuerdo con la política de la empresa. Registra con precisión los minutos de tratamiento de los servicios prestados según todos los requisitos reglamentarios y clínicos.

· Participar en las capacitaciones, servicios y conferencias requeridos por las instalaciones y la compañía.

· Capaz de supervisar y apoyar a los asistentes de fisioterapeuta; ayudantes de rehabilitación y/o estudiantes.

· Cumplimiento de la política y los procedimientos requeridos por la empresa y del código de conducta.
