Licensed Practical Nurse (LPN) Duties and Responsibilities:

  • Provides licensed care to assigned residents as ordered by physician and in accordance with facility, federal, state and local standards, guidelines and regulations.

  • Provides required resident assessments/observations and interventions, reviewing care plans to ensure appropriate care. 

  • Takes and records residents blood pressure and vital signs, isters catheter care, isters IV therapy to residents as ordered within license limitations, and provides respiratory care, e.g. oxygen, nebulizer treatments. 

  • Charts nurse’s notes in an informative and descriptive manner that reflects the care provided as well as the resident’s response to the care. 

  • Ensures the resident’s environment is as free from accidents and hazards. Immediately corrects/addresses potential safety risks as prudent and warranted. 

  • Monitors and directs non-licensed ancillary services as deemed necessary to meet professional standards and maintain the highest practicable level of resident care.

  • Communicate/report information to residents, resident families, physicians, guardians, governmental agencies, and facility personnel as required by facility policy and State/Federal guidelines. 

  • Ensures that all aspects of resident care plans are implemented and maintained. Make recommendations for care plan changes to the IDT per professional standards. 

  • Ensures compliance with infection control standards. Immediately corrects/addresses identified instances of non-compliance. 

  • Follows and adheres to company policies and procedures regarding HIPAA (Health Insurance Portability and ability) compliance. 

  • All other duties, as assigned.

Licensed Practical (LPN) Nurse Qualifications and Skills:

  • Excellent written and verbal communication skills in English. 

  • Ability to multi-task and remain calm under stress. 

  • Analytical and problem-solving skills. 

  • Understands and follows infection control standards, policies and procedures.

  • Must be able to adapt to a flexible work schedule and occasional overnight travel.

  • Ability to relate positively, effectively, and appropriately with residents, families, community , volunteers and other facility staff.

Licensed Practical Nurse (LPN) Education and Experience:

  • Nursing Degree or diploma from an accredited College or University. 

  • Possess a current license to practice in the State as a Licensed Practical Nurse. 

  • 1 year of experience preferred.

  • Long-Term care experience preferred.

Physical Requirements:

  • Prepare & deliver snacks, etc., as instructed. Deliver food carts, trays to designated areas. Serve food and drinks in the dining room as instructed. Sweeping & mopping floors in the kitchen/dish room as instructed by supervisor.
  • Dishwashing.

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements below are representative of the knowledge, skill, and/or ability required.

  • Must be able to read, write, speak, and understand English.
  • Must maintain the care & use of supplies, equipment, the appearance of work areas, and perform regular inspections of food service areas for sanitation, order, safety and proper performance of assigned duties.

Primary Duties:

  • Knowledge of EDI order processing preferred
  • Experience and understanding of ERP systems preferred

ed Nurse (RN) Duties and Responsibilities:

  • Provides licensed care to assigned residents as ordered by physician and in accordance with facility, federal, state and local standards, guidelines and regulations 
  • Ensures assigned work area (i.e. nurse station, medication cart, medication room, etc.), resident care rooms and treatment areas are maintained in a clean and sanitary manner.
  • Takes and records residents blood pressure and vital signs, isters catheter care, isters IV therapy to residents as ordered within license limitations, and provides respiratory care, e.g. oxygen, nebulizer treatments. 
  • Charts nurses notes in an informative and descriptive manner that reflects the care provided as well as the resident’s response to the care. 
  • Ensures the resident’s environment is as free from accidents and hazards. Immediately corrects/addresses potential safety risks as prudent and warranted. 
  • Monitors and directs non-licensed ancillary services as deemed necessary to meet professional standards and maintain the highest practicable level of resident care 
  • Communicate/report information to residents, resident families, physicians, guardians, governmental agencies, and facility personnel as required by facility policy and State/Federal guidelines. 
  • Ensures that all aspects of resident care plans are implemented and maintained. Make recommendations for care plan changes to the IDT per professional standards. 
  • Ensures compliance with infection control standards. Immediately corrects/addresses identified instances of non-compliance. 
  • Follows and adheres to company policies and procedures regarding HIPAA (Health Insurance Portability and ability) compliance.

SUMMARY: The Cook is responsible for food preparation in accordance with current applicable federal, state, and local standards, guidelines and regulations, with our established policies and procedures, and as may be directed by the Dietitian and/or Director of Food Services, to assure that quality food service is provided at all times.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Review menus prior to preparation of food.
  • Assist in developing, implementing and coordinating department policies, procedures, and food service manuals.
  • Interpret department policies and procedures to personnel, residents, visitors, family , etc., as necessary.
  • Assist in planning regular and special diet menus as prescribed by the attending physician. Assure that food is available for preparation by cooks.
  • Coordinate food services with other departments as necessary.
  • Work with the facility’s dietitian and implement recommended changes as required.
  • Ensure that all food service procedures are followed in accordance with established policies.

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements below are representative of the knowledge, skill, and/or ability required.

  • One (1) year dietary/food experience in a hospital, nursing care facility, or other related medical facility preferred but not required.
  • Must have State Food Service Sanitation certification.
  • Must be able to read, speak and write the English language in an understandable manner.
  • Must function independently, have flexibility, personal integrity, and the ability to work effectively with the residents, personnel, and agencies.

 

 

PHYSICAL DEMANDS: The physical demands are representative of those that must be met by an employee to successfully perform the essential functions of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  • Must be able to move intermittently throughout the workday.
  • Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.
  • Must be able to taste and smell food to determine quality and palatability.
  • May be necessary to assist in the evacuation of residents during emergency situations.

The Operations Manager will effectively manage incoming BT training and center schedule, maintain the center facility, keep supplies up to date and within local code, as well as additional tasks to include social media updates and special events.

WORK SCHEDULE: Mon-Fri 8:00am - 4:00pm/10:00am - 6:00pm

 

PRIMARY DUTIES & RESPONSIBILITIES

  • General facilities management relating to the clinic or township.

  • Coordinating behavior therapist hiring and onboarding for the clinic.

  • Facilitates new hire training and orientation.

  • Creating and maintaining center schedule for behavior therapists.

  • Coordinates shift coverage and schedule changes.

  • Updating and maintaining all supply orders for facility and client needs.

  • Manages general reception tasks relating to the front desk.

  • Prepares weekly operational reports for Center Director.

  • General of the behavioral therapist's team.

ADDITIONAL REQUIREMENTS

Training/BT/Schedule

  • Updating the system keeping client information current and accurate on a consistent basis.

  • Responding to client inquiries in a timely fashion.

  • Interviewing, hiring, and ensuring that the appropriate training has been scheduled for employees.

  • Assists with opening and closing of the center.

  • Provide to RBT/BTs to promote job satisfaction & engagement.

  • Provide regular check-ins throughout the day with staff to ensure well-being.

  • Facilitates coverage for time off requests and cancelations by clients or staff.

  • Complete check-in and check-out procedure via pre-established check in/out process.

  • Maintain a positive, problem solving and ive mindset.

  • Communicate openly and directly with management regarding concerns or ideas.

  • Adhere to and promote professional standards of KAS.

  • Participate in ongoing training as assigned.

  • Participate in meeting as assigned by the Clinical Director (Team Meetings and 1:1).

Facility

  • Maintain compliance with safety regulations for staff and clients

  • Maintain cleanliness and upkeep of the clinic

  • Ensure and maintain emergency protocols.

  • Coordinate maintenance & repairs with approved vendors

  • Ensure clinic is sanitized regularly according to schedule with Extreme Clean cleaning company.

  • Assist with opening and closing preparations.

  • Stock rooms daily with necessary supplies

  • Establish and maintain a safe, healthy learning environment

  • Ensure all PHI is stored in compliance with HIPAA regulations and strictly viewed and handled only in a HIPAA compliant work environment

Supply

  • Manage and maintain inventory of supplies

  • Tracks need for supplies and communicates with the Clinical Director (both for site and BCBA programming: including clinical, general, office, and PPE supplies).

Additional

  • Assist when assigned in Facebook Lives and lifestyle posts

  • Coordinating special events for the clinic: birthdays, holidays, in accordance with set-forth budget

  • Assist's clinicians with program material creation.

  • Assist's with organizing and maintaining storage closets, cabinets, and drawers.

  • Assists with sanitization of toys and materials.

  • Responds to walkie talkie requests in from center staff in a timely manner.
